Post-Acute Care Specialty Programs

Inpatient Rehabilitation Facilities (IRF)

These rehabilitation facilities serve medically fragile patients who require an intense level of physical, occupational and speech therapy in an inpatient hospital setting. DRG exempt IRFs are specially licensed and Medicare certified. This includes highly regulated and close monitoring by CMS. The prevailing goal for these patients is to return to the community at the highest level of independence. This is accomplished with an intensive treatment program including 3 hours of therapy a day and specialized rehabilitation nursing spanning a 12-14 day average length of stay. Skilled Nursing Facilities (SNF)

Hospital based Skilled Nursing Facilities, utilize medical, nursing and therapy interventions to evaluate and manage the healthcare needs of patients. The specific types of skilled interventions vary widely. Examples are: skilled observation with medical supervision; complex medical management; physical rehabilitation with skilled nursing services and; medical supervision. Hospitals can optimize the post acute care continuum by having collaborative SNFs and IRFs. The majority of SNF patients receive some form of therapy daily yet they do not require nor tolerate the intensity of 3 hours of therapy a day.

Inpatient Psychiatric Facilities (IPF)

Behavioral Health units are specially designed and operated for adult and/or geriatric patients over 65 who require psychiatric treatment and, typically, medical or pharmacological intervention. These units are DRG exempt from Acute PPS and must comply with very specific CMS regulations to maintain exemption status. Length of stay for the geriatric psychiatric population is approximately 14 days to realize the full benefits of psychotropic medications and behavioral management of symptoms. The goal of adult and geriatric behavioral health programs is to return the patient to a safe environment of the patient’s choice with symptoms controlled through newly learned coping mechanisms and/or appropriate medications. Long Term Acute Care Hospitals (LTCH)

These post-acute care facilities offer services for medically complex patients who require extended recuperation periods and a number of pharmaceutical, medical and therapeutic interventions in an interdisciplinary setting for optimal outcomes and patient/family disease management intervention and education. They may frequently require dialysis and respiratory support along with the medical management of their complex chronic medical conditions. The lengths of stay are generally over 25 days. The goal of LTCH care is to return the patient to the highest level of independence while treating their complex medical conditions and offering the highest quality of life options available. MileStone has led the industry in providing flexible interface options to client hospitals including lease, joint-venture, management or consultation.

Swing Bed Program

Rural hospitals,  as delineated by the U.S. Census Bureau that have been approved by CMS to furnish swing bed services, may use any acute care bed within the hospital for the provision of swing bed care.  This means that the hospital can swing the patient from acute care services to swing bed skilled services; and, the patient does not need to change beds or locations within the facility.  This provides the rural patient / Medicare beneficiary access to post-acute skilled level of care. 
The advantage of the rural swing bed program is that the patient continues to receive medical as well as skilled care and services in their home community.

Outpatient Therapy Services

Outpatient therapy clinics offer physical, occupational and speech therapy care that is customized for each patient.  The clinics treat patients from age 1 day to 99 years plus.  Outpatient services cross all medical specialties:  orthopedics and musculoskeletal, neurologic, industrial medicine, sports medicine, cardiac, rheumatic diseases, trauma, speech and swallowing disorders and pediatrics.  Care includes:  health and wellness, injury prevention, injury care, pain management, pre-op and post-op care, chronic disease management and patient education.  Outpatient services can be paid for by: Medicare, Medicaid, VA, commercial insurance companies, worker compensation and client cash pay.
